How will the results be used?

Take your time and answer the questions as honestly as possible. Apart from evolving the result of the evaluation for you - we do not use or share the information given on this sheet.

You must know that all questions are general and do not intend to diagnose, prevent, treat or cure any disease.  If you have any health concerns, please contact your doctor or dietitian or applicable medical professional.

#1 Do you eat together as a family at least 3-4 times a week (ideally at least once a day)?
#2 Is fish on the menu at least twice a week (fresh, canned, smoked or frozen fish, seafood or shellfish)?
#3 When you are shopping, do you check the food labels before choosing a product?
#4 Does everyone in your family eat breakfast every day?
#5 Does every family member eat fruits/vegetables at every meal?
#6 Do your family have most of the meals in front of the TV?
#7 Does your family eat pre-packed meals / ready meals / takeaway food more than once a week?
#8 Are sweets (candy, lollies, chocolate, biscuits etc.) often used as a reward for good behaviour in your family?
#9 Is there one or more family members who are struggling to reach or maintain their ideal weight?
#10 Do you have a routine to plan family meals and when you are shopping, do you follow a shopping list?
#11 Do you eat deep-fried food more than once a week?
#12 Is one (or more family member) suffering from hair loss, swollen feet and muscle weakness?
#13 All family member drink mainly water as a drink (more than 75% of the fluid intake is water)?
#14 Do you know what a “balanced meal” means?
#15 Is drinking water always available and easily accessible for everyone (even small children) in your house?
#16 Is one or more family members suffering often from constipation or diarrhoea?
#17 The fat used at cooking is mainly olive oil, rapeseed oil and/or sunflower oil?
#18 Does your family eat only white, highly refined bread, bakery products (like white bread, Danish pastries, muffin, brioche, white flour wraps and flat breads, white pasta and white rice) instead of whole grain products?
#19 Is home-made meal on the menu at least 5-7 times a week?
#20 All family members eat 5 or more portion of fruit and vegetable every day (including fresh, dried, canned and frozen)?
#21 Most of the days every family member eat 3 main meals and 2-4 smaller snack?
#22 Snacks in your family usually mean fruit juice, sweets, crisps, biscuits, muesli/granola bars, chocolate bars or cakes?
#23 Does your family eat red meat  2 or fewer times per week?
#24 Alcohol consumption for adult members of the family usually do not excess the 2 units/week?
#25 Does all family member take a multivitamin supplement regularly?
